Improving care for people living with chronic conditions is central to the mission of the Johnson & Johnson Diabetes Institute. One new initiative to improve chronic care through primary care is a model known as the “Patient-Centered Medical Home.” The Institute’s own Teresa Pearson, MS, RN, CDE, FAADE is an expert in helping organizations meet the standards set by the National Committee for Quality Assurance (NCQA).
